Three separate problems in cover replacement, all now fixed.
- While a new cover was awaiting review, the track showed a broken image
- An approved cover was not actually put live
- A rejected cover wiped the artwork you already had, instead of reverting to it
If you replaced a cover recently and something looked wrong, it should be correct now. If it is not, tell us in the comments.
